Crystal Clear is committed to protecting your Practice’s privacy and developing technology that gives your Practice and your patients the most powerful and safe online experience. This Statement of Privacy applies to the Crystal Clear website and governs data collection and usage. By using the Crystal Clear website, your Practice consents to the data practices described in this statement.
Collection of your Personal Information
Crystal Clear collects personally identifiable information, such as your patient’s and Practice employee’s e-mail address, name, home or work address or telephone number. Crystal Clear also collects anonymous demographic information, which is not unique to your Practice or your patient, such as zip code, age, gender, preferences, interests and favorites.
There is also information about your Practice’s computer hardware and software that is automatically collected by Crystal Clear. This information can include: your Practice’s IP address, browser type, domain names, access times and referring website addresses. This information is used by Crystal Clear for the operation of the service, to maintain quality of the service, and to provide general statistics regarding use of the Crystal Clear website.
Please keep in mind that if your Practice or patients directly disclose personally identifiable information or personally sensitive data through Crystal Clear public message boards, this information may be collected and used by others. Note: Crystal Clear does not read any of your patient’s or your Practice’s private online communications.
Crystal Clear encourages your Practice to review the privacy statements of websites your Practice chooses to link to from Crystal Clear so that your Practice can understand how those websites collect, use, and share your Practice’s information. Crystal Clear is not responsible for the privacy statements or other content on websites outside of the Crystal Clear and Crystal Clear family of websites.
Use of your Personal Information
Crystal Clear collects and uses your Practice’s personal information to operate the Crystal Clear website and deliver the services your Practice has requested. Crystal Clear also uses your Practice’s personally identifiable information to inform your Practice of other products or services available from Crystal Clear and its affiliates. Crystal Clear may also contact your Practice via surveys to conduct research about your Practice’s opinion of current services or of potential new services that may be offered.
Crystal Clear does not sell, rent or lease its patient lists to third parties. Crystal Clear may, from time to time, contact your Practice on behalf of external business partners about a particular offering that may be of interest to your Practice. In those cases, your Practice’s unique personally identifiable information (e-mail, name, address, telephone number) is not transferred to the third party. In addition, Crystal Clear may share data with trusted partners to help us perform statistical analysis, send your Practice’s email or postal mail, provide patient support, or arrange for deliveries. All such third parties are prohibited from using your Practice’s personal information except to provide these services to Crystal Clear, and they are required to maintain the confidentiality of your Practice’s information.
Crystal Clear does not use or disclose sensitive personal information, such as race, religion, or political affiliations, without your Practice’s explicit consent.
Crystal Clear keeps track of the websites and pages our patients visit within Crystal Clear, in order to determine what Crystal Clear services are the most popular. This data is used to deliver customized content and advertising within Crystal Clear to patients whose behavior indicates that they are interested in a particular subject area.
Crystal Clear websites will disclose your Practice’s personal information, without notice, only if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the edicts of the law or comply with legal process served on Crystal Clear or the site; (b) protect and defend the rights or property of Crystal Clear; and, (c) act under exigent circumstances to protect the personal safety of users of Crystal Clear, or the public.
Use of Cookies
The Crystal Clear website uses “cookies” to help your patients and your Practice personalize their online experience. A cookie is a text file that is placed on the end-user’s hard disk by a web page server. Cookies cannot be used to run programs or deliver viruses to computers. Cookies are uniquely assigned to your Practice and your patients’ computers, and can only be read by a web server in the domain that issued the cookie to your Practice or your patient.
One of the primary purposes of cookies is to provide a convenience feature to save your Practice and patients time. The purpose of a cookie is to tell the web server that your Practice or patient has returned to a specific page. For example, if your Practice or patient personalize Crystal Clear pages, or register with Crystal Clear site or services, a cookie helps Crystal Clear to recall your Practice’s or patient’s specific information on subsequent visits. This simplifies the process of recording your Practice’s or patient’s personal information, such as billing addresses, shipping addresses, and so on. When your Practice or patient returns to the same Crystal Clear website, the information your Practice or patient previously provided can be retrieved, so your Practice or patient can easily use the Crystal Clear features that your Practice or patient customized.
You have the ability to accept or decline cookies. Most web browsers automatically accept cookies, but your Practice or patient can usually modify the browser setting to decline cookies if preferred. If your Practice or patient chooses to decline cookies, your Practice or patient may not be able to fully experience the interactive features of the Crystal Clear services or websites your Practice or patient visits.
Additionally, we use third-party advertising companies (including Google and Adroll) to serve ads which may have referred your Practice or patient to our website. These companies may use information (not including name, address, email address, or telephone number) about your Practice or patient’s visits to this and other websites in order to provide advertisements about goods and services of interest to your Practice or patient. If your Practice or patient would like more information about this practice and to know your choices about not having this information used by Google, click here.
As you browse Crystal Clear Digital Market, advertising cookies will be placed on your computer so that we can understand what you are interested in. Our display advertising partner, AdRoll, then enables us to present you with retargeting advertising on other sites based on your previous interaction with www.crystalcleardm.com. The techniques our partners employ do not collect personal information such as your name, email address, postal address or telephone number. You can visit this page to opt out of AdRoll and their partners’ targeted advertising.
Security of Your Personal Information
Crystal Clear secures your Practice and patient’s personal information from unauthorized access, use, or disclosure. Crystal Clear secures the personally identifiable information provided on computer servers in a controlled, secure environment, protected from unauthorized access, use, or disclosure. When personal information is transmitted to other websites, it is protected through the use of encryption, such as the Secure Socket Layer (SSL) protocol.
